ALL player's shall play from the same yardage.

Open to any current legal residents of the state of Georgia.

A player may be required to show proof of residency.

If asked a player must present a Non-expired Georgia driver's license, permit or identification card AND two of the below items as proof that he or she resides in Georgia and must provide documentation of a valid Georgia residence address.

1. Utility bill issued within the last thirty (30) days of the date of Championship/Qualifier.

2. Bank statement issued within the last thirty (30) days of the date of Championship/Qualifier.

3. Current valid rental contracts and/or receipts for payments made within the last thirty (30) days for rent payments of the date of the Championship/Qualifier.

4. Employer verification, including, but not limited to, one of the following:

a. Paycheck or paycheck stub within the last thirty (30) days of the date of Championship/Qualifier.

b. Letter from employer on company letter head with Georgia address on letter head.

c. W-2 for current calendar year.

5. Health insurance statement.

6. State of Georgia or Federal income tax return for current calendar year.

7. Annual social security statement for current or calendar year.

8. Medicare or Medicaid statement.

9. Mortgage, payment coupon, deed, or property tax bill for current calendar year.

REFUNDS:
1. Withdraw before the entry deadline (10:00 a.m. Friday, July 14, 2017) and you will receive a full refund less a $10 administrative fee.

2. Withdraw after the entry deadline, but before the first group tees off in the championship, a player will receive a full refund less a $75 administrative fee.

3. Withdraw after the first group tees off in the qualifier, a player will forfeit the entire entry fee.
Practice rounds MUST be arranged with the host facility.

Your first practice round is $25 and each additional practice round is at the discretion of the club.

You must call the golf shop and set-up your practice round.

Practice rounds are available on Saturday & Sunday, July 25-26, 2015 AFTER 3:00 p.m. only.

The consumption of alcoholic beverages is strictly prohibited during a competitors stipulated round in any Georgia PGA tournament. Any player violating this policy will be removed from the competition immediately.
Amateurs may wear shorts during the qualifier and the Championship.

ALL PGA Professionals and Non-PGA Professionals will be required to wear long pants in both the qualifier and Championship.
Distance-Measuring Devices –
A player may obtain distance information by using a device that measures distance only. If, during a stipulated round, a player uses a distance-measuring device that is designed to gauge or measure other conditions that might affect his play (e.g., gradient, wind-speed, temperature, etc.), the player is in breach of Rule 14-3 for which penalty is disqualification, regardless of whether any such additional function is actually used.
A player may be assisted by a caddie, but he is limited to only one caddie at any one time.

Participants are required to use a golf car. The fee for the golf car is included in the entry fee. All players’ bag must be on a golf car and only two (2) golf cars per-group will be allowed.
